Insurance question: Does anyone know how long

after switching to a new health insurance company you have to wait before you could have hardware removed from an old fracture?  Since it hasn't been 90 days without insurance, would it still be preexisting and/or have to wait a year or two, or can it be done immediately since there was no lapse in insurance?
