What's the grossest thing you ever heard while transcribing?

1. Doc with sinus problems snuffing back sinus drainage.

2. Phone sex when the doc forgot he was dictating and took a call from his girlfriend.

This had nothing to do with the dictator. It was about the patient's care or lack thereof.

The doctor was dictating a brief patient history within an operative report. He said that the patient had undergone 2 unsuccessful ray amputations and was now status post a right below-knee amputation. The patient had been residing at the hospital's extended care facility for wound care and physical therapy. However, when the patient presented to Physical Therapy one day, the therapist noted his left foot was malodorous. They removed his sock and found that the foot was gangrenous and there were maggots coming out of it. I was stunned. Apparently, so was the doctor because I heard his voice catch when he dictated what had happened.

How does this happen while someone is an inpatient? I still shudder when I think of this.

Maggot therapy is also known as maggot debridement therapy (MDT), larval therapy, larva therapy, larvae therapy, biodebridement or biosurgery. It is a type of biotherapy involving the introduction of live, disinfected maggots (fly larvae) into the non-healing skin and soft tissue wound(s) of a human or animal for the purpose of cleaning out the necrotic (dead) tissue within a wound (debridement) and disinfection. It was long believed that the debridement is selective on necrotic tissue but new literature first questioned that belief.[1] A recent clinical study in France showed non-selectivity of maggot action as wound surface increased over treatment.

Maggots eat dead flesh, so it's not clear if these maggots were planted on purpose for therapy, or if they appeared on their own. If they appeared on their own, obviously this person had not been receiving proper treatment, as it takes a while for the maggots to appear on their own.

Ever have dead, rancid meat in the garbage pail? That's where the maggots come from.

The gangrene and the maggots were first discovered by a physical therapist while the person was an in-patient. The patient had been transferred directly from the hospital after having a below-knee amputation of the contralateral limb. My horror comes from the fact that, despite being there for wound care treatment as well, no doctors, nurses, or PCAs checked or noticed. That is unacceptable.

Maggots do not arise out of nowhere. There have to be eggs from which the maggots hatch. If you put a piece of dead meat under cover and no flies can get to it, maggots will not appear out of nowhere. Oh my goodness.

That someone had food stuck in her throat for a week - and smells a rotting smell

She just left it there and a week later went to the ER. The entire report was pretty descriptive. Needless to say I was not hungry for lunch that day.

I don't know what's going on in my dictation, but if I had to take a wild guess, I'd say the dictator is getting down and dirty with someone... hmm.
