These are just a few...  I just can't imagine what comes out of voice recognition software for some of these folks.

 

She gets it on and off ever since she fell on her left hip in 2008 about twice a year. 

 

It is a little peculiar about the patient who has her symptoms worse during the day instead of usually first thing in the morning with the drainage is when it is the worst.  

 

Apparently the wife wanted her daughter who, according to the patient, he does not wish to do anything with her and he does not wish to go down to the wife. 

 

She is complaining of an earache in her ears. 

 

He also on his left wrist on his lower side on the side toward the ulnar but not in the wrist itself but just proximal to that, he has another little fatty, 2 cm type of a tumor. 

 

It is most likely some food poisoning so I told her – that was after she ate some nuts that were old – to go through all of her food and throw out all the old stuff and be sure; about once or twice a year she gets this so there is just some food sitting around too much. 

 

On Monday she had some Red Bull.  She admits to half of an 8 ounce can and it also has ginseng in it, which is bad for Abilify.  It is contraindicated as well as taurine.  The patient is now very – her arms are shaky and jerky and tight.  Checking her arms, they are kind of rigidity and a little cogwheeling. 

 

For her spasms she will just have to wait until she gets the clonazepam and use time and it should go away. 

 

He is very hard of hearing but his skin is very clammy. 

 

She says she has been coughing and has a throat.

 

It is getting a little bit worse so I would like you to see Dr. Doc, the hematologist, to see if he has any recommendations that could be aggravating this problem. 

 

She is still having now she describes it as burning on both sides.  I had seen her in the past in the vaginal area but I had seen her in the past with some itching and I really could not find nothing. 

 

She has not had any lab but since she is nonfasting we will skip that for a long time. 

 

The doctor thinks her bladder does not empty well and recommends she three times a day empty again but she has done that.  

 

No pain in the growing (groin). 

 

She would like her medicines refilled as it sounds like she has been trying to get 90-day supplies, which the pharmacy has just been asking for refills, which sounds like we have come up with the reason for the miscommunications because she was trying to get 90-day refills. 

 

He has a sister living and well.  She is two years younger than the patient other than a little back trouble. 

 

Her last chest x-ray was fine so we did not repeat that just a month ago. 

 

Her left hand went away. 

 

He also needs basic lab work drawn and we ordered a CBC, CMP, TSH, lipids, and PSA and we will get those results back when he is fasting.  

 

In visiting with the nurse on the patient, he drives around on a little scooter.  The patient is not in his room.  He is not in therapy either.  We cannot find him today but we changed his medicines as above.  We will keep a close eye on him.   (can’t find the man but they’re going to keep a close eye on him…  right….)

 

She does not have any husband and is not pregnant in any way.

Thanks for sharing. I've heard a few crazy ones, too.

The patient is an elderly white male lying comfortably in bed accompanied by his sister.

The patient has no difficulty coughing up or swallowing blood.

She has no difficulty with her memory except for the parts she cannot remember.

His memory is impaired, especially for past events.

I've got the local respiratory creeping crud.  Saw the doc and he prescribed prednisone.  The pharmacist had to call him back today.  Prescription was written for 20 tablets, "1 on day one then 5, 4, 3, 2, 1 on successive days once a day".  I'm thinking that this makes no sense at all.  All of this MT work pays off.  He meant 6 on day one then 5 on day 2, 4 on day 3, etc., but the rx was written exactly as above.  How do common folk ever know what the ...
