The math is really quite simple, and there ain't no free lunches.

My doc is 58, will take retirement end of year - and no new Medicare patients until then.

He sees about 150 Medicare patients a month now. Wonder where they'll go since he's the only specialist in his field for 30 miles in any direction and he's going to sell his valuable downtown office to a commercial developer for more than he could get for the practice.

Is it unusual for a physician to retire at 58? Don't you mean no new patients (not just Medicare). Why would he take any new patients only to be left with no physician because he sold the building without which the practice cannot be sold to another?

There is no such thing as a "practice entity" that exists as some metaphysical reality apart from physicians and the facilities in which they deliver healthcare. Do the patients still exist? Yes. Does the practice? No.

And whether some physicians retire at 58 or not is hardly the point - this PARTICULAR physician had NOT been planning to do so.

You're really in enormous denial about the deleterious impacts that we already know quite well this healthcare bill will have on the system, aren't you? Well, the great thing is that you'll have a very, very long time to regret that you refused to see them.

For starters, you should know the difference between tangible and intangible assets. These can be grouped into two broad categories: physical and nonphysical. The former includes real estate and leaseholds, medical equipment and furnishings, and accounts receivable. The latter includes goodwill, restrictive covenants, and your staff.

A medical practice is a business entity, no different from a grocery store. When the store is sold to create a parking garage, it ceases to exist. You might try to buy carrots from the parking attendant all day long, but they'll come and snatch you up with butterfly nets for a long, long rest where you can't hurt anybody or yourself.

When this physician closes his office, sells it and dismisses his employees, THE PRACTICE IS GONE!

and look on the internet and you can find how a medical "practice" works. Pay particular attention to the part about the physician's reputation.

Try thinking about this too:
If, as you say, a "practice" is neither tangible or intangible just how are "practices" sold (which happens every day)?

If, as you say, a "practice" is a business just like a grocery store, how can it not have either tangible or intangible assets (unless it failed which doesn't appear to be the case here)?

I think they have the votes. The so-called "fence-sitters" will be allowed to remain in the well of the House until the end of the vote when Pelosi sees how many of them she needs to hit 216. She'll take just enough of them to pass the bill, and allow the others to vote "No" so as to try to save their sorry arses in November. She's done this before.
