Under Hawaii's Prepaid Healthcare Act of 1974, businesses in the state are required to offer health insurance to employees who work more than 20 hours per week. As a result, Hawaii has consistently had one of the lowest uninsured rates in the country.

of the residents of Hawaii are covered under insurance through their employer. Because of the few people needing to use the state exchange it was not workable financially. Those people will no w use the federal exchange. Medicaid eligibility has also been expanded in the Aloha state.
