they hired you in the first place if you have disabilities with balance, a torn rotator cuff, and trouble following and remembering instructions. Just write a letter giving them notice due to the fact that you are not physically capable of the job. Hand deliver it to their office in person and turn in the 3 shirts with their logo. It sounds like there is a bit of age discrimination going on also. Just hold your head high, admit you cannot do the job, and resign. No need to be afraid. What does the contract say anyway and what can they do? You just can't handle the job, and they have to accept your resignation. Just make sure you get paid for the work you have done. If you don't, get in touch with your state's DOL about that.
