You might try some kind of foam cushion on the earbud. The reason for this is you can then remove that cushion and wash it with bleach in order to sterilize it. Also, after washing the earbuds themselves, rinse THEM with bleach water. Now your ears: Wash them at least three times a day with Dial or some antibacterial soap. Then dry very good. Perhaps try hydrogen peroxide, dry again. Then put a little antibiotic cream. But before you put those earbuds back in, make sure your ears are dried of any liquid. Headphones are disgusting. I refuse to put anyone else's earphones/earbuds in my ear because no one washes them. By the way, do you use under-chin headsets? Those may not be as irritating for you - but the foam pads they come with need to be washed/sterilized every month or so.
I used to just throw those foam pads away, but now I wash them in the washing machine and rinse in bleach water. I have at least 15 extra pair. And as soon as I feel my ears getting irritated, I start the aggressive treatment I suggested above. Long post, but this is something never discussed during my transcription training.
