I believe the doc should help document - preauthorization

Not all conditions have to be life threatening, but I think the doc needs to document why it is "necessary" (and not cosmetic). I think it is a good practice to always get pre-authorization (as with private insurance).

My doc asked me if a mole I would like removed "has changed--wink, wink." If a mole changes, it is sent to the lab for testing and it is covered. If I want it removed for cosmetic reasons, I will pay. Actually, I think I have paid the insurance companies plenty over my lifetime without hardly ever using it. So, I do think that mole has changed a bit.
